Records and Transcripts

Records Request Policy

Who Can Request Records

  • Only parents/guardians (on record with the Registrar) may request records for any child who attended or attends our school and is under 18. Our Parental Release Form must be submitted.
  • Only the student may request their academic records once they reach the age of 18. An email with identifying information must be submitted.

Submitting Your Request

  • Please allow 5-7 working days to process your request.
  • All recommendation forms must be submitted to the Registrar’s office directly for proper handling.

Delivery Methods

  • Official packets will be sent via regular mail through the United State Postal Services
  • Official packets can be emailed directly to the intended 3rd party recipient
  • Unofficial copies are only emailed out to the requester
  • We do not offer UPS/FedEx delivery methods

Requests we don’t handle

  • The College Counseling office handles all records for college purposes and there is no fee to obtain academic records from Miami Country Day School.
  • We do not supply any other schools’ records in our standard records packets. 
  • We do not supply SAT or ACT scores on behalf of any former student, current student or alumni – that is the student’s responsibility.

Types of Requests We Handle

  • Alumni transcripts
  • Recommendations for transfers and summer programs (forms must be provided to the Registrar’s office)
  • Proof of enrollment letters
  • Community Service letters
  • Education consultant forms
  • Court ordered subpoenas
  • Transcript & Report Card packets (official and unofficial copies)
  • Recruitment
  • Social Security Administration forms
  • Good student driving discount forms
  • Formal Education Plans
  • Graduation verification for employment

Our Standard Records Packet

The standard records packet we provide includes:
  • Two academic years’ report cards; current and previous (Lower and Middle School)
  • A transcript (Upper School)
  • Standardized test scores (ERB)
  • S- Health records (copies of DH680 and DH3040 State forms)

  • Alumni

    Email or fax the Director of Records and School Registrar with the following information:
    • Your full name
    • Your date of birth or last four (4) digits only of your Social Security Number
    • Your date of graduation
    • Where you want the transcript sent; including School/Program Name, Mailing Address or Email Address and “To the Attention of Name/Department”

  • Transfers or Summer Programs

    We require a Parental Release form for all records requests for current or former students, who are younger than 18 years of age. You may complete this form online or print and mail/fax/email the form to the Registrar’s office.
    Please complete this release form by entering the students name, DOB and current grade level. Check on the records needed from the list provided on this form, enter the school name/program and address/email and in Special Handling indicate any special needs or if recommendations are needed and from whom. Forward those forms to the Registrar’s office via fax, email or in-person.

  • Other Requests

    Other requests include the following:
    • Proof of enrollment letters
    • Community service letters
    • Recruitment authorization
    • Social Security
    • Administration forms
    • Good student driving discount forms
    • Formal Education Plans
    • Education consultant forms
    We require a Parental Release form of these types of requests for current or former students. Only parents/guardians of record may make the above requests either online or print and mail, fax or email the form to the Registrar’s office.
    Please complete this release form by entering the students name, DOB, and current grade level. Check on the records needed from the list provided on this form, enter the school name/program and address/email and in Special Handling indicate any special needs or if recommendations are needed and from whom. Forward those forms to the Registrar’s office via fax, email or in-person.
