After July 1st, be sure to upload the requested health forms. The health forms must be submitted by August 1st to Magnus Health in order for your child to attend classes on the first day of school. A link is available on the "Resources" tab in your MyCountryDay account; click the "Magnus Health" tile to access. The health forms need to be completed and signed by a Florida licensed healthcare provider.
